The basic underlying idea that Kafka seems to be dealing with in this story is the utterly irreconcilable contradiction between the manner in which a person views himself as opposed to the manner in which the outside world views him. Nothing could be more disgusting and reprehensible to outside observers than a gigantic cockroach - and his family, even his dear sister, in spite of all their love for him, can view him as little more than that - yet the protagonist (Gregor) insists on viewing himself as a human being, albeit presently in a disadvantaged state. All of his actions are misunderstood by his family, because he is under the delusion that they still perceive him as a human being and he acts accordingly, while in truth they view him as a horrible, monstrous pest, and therefore apply a totally different interpretation to all of his actions.
It is all summed up in one important passage, near the end - his sister comments that it cannot be their son and brother who is locked in the room, because their brother wouldn't have put them through all this grief, but would have realized what a horrible burden he was and left them alone, not remaining in the house to torment them. In other words, had Gregor really fathomed that he was perceived by others as an insect and not a human, he would have left them in peace - he remained because he still perceived himself as human, despite what his senses told him to the contrary.
I'm pretty sure that when Kafka called it "The Metamorphosis", he was referring principally to the psychological metamorphosis that Gregor undergoes in the course of the story, as he becomes more and more aware of the horrible truth of how his family now looks at him. When he comes to fully realize that - when he has confirmed beyond all doubt that they view him as nothing but a monstrous pest - that he has no human identity whatsoever in the real world - it all exists only in his imagination - then he actually does cease to exist in body also - and breathes his last.
